ROLE SUMMARY:

  • To work as an experienced member of staff in the Community Mental Health Team. This is a multi-disciplinary team, with a single manager operating in a locality where there is a high degree of integration between all mental health services.
  • Conducting assessments, providing high quality treatments, care co-ordination, long term casework, purchasing of services and supervision of staff are some of the core activities of the post. Working collaboratively with other disciplines and other teams on the locality are essential in order to ensure a comprehensive service is provided to the local population. As a senior member of the team the postholder will need to act as role model and be able to manage high levels of responsibility.

Main Duties & Responsibilities:

  • The post holder will ensure that each individual plan is developed in consultation/agreement with the patient and informed by the wider multi-disciplinary team.
  • Will be responsible for identifying the specific interventions and skills required from the multi-disciplinary team to deliver the plan of care and expected health/social outcomes which are evidenced based and lead to recovery and minimise risk.
  • Will be responsible for devising a crisis plan with each individual patient and alerting the Urgent Care services as required.
  • Will have a recognised qualification in Psycho-Social Intervention (PSI)
  • Will operate within the principles of recovery and as such support each patient to develop a Wellness Recovery Action Plan and Recovery STAR and reduce dependence on Secondary Mental Health Services enabling a planned integration into local community facilities and then discharge as part of the patient’s recovery plan.

Essential Skills:

  • Ability to work autonomously and set own priorities within caseload management.
  • Ability to carry out high quality assessments and treatment interventions to individuals and groups.

Qualification:

  • Registered for practice through NMC.
  • Further relevant qualification in the field of mental healthor equivalent experience.

Experience required.

  • Minimum two years post-qualification experience of working with adults with mental health problems.
  • Experience of multi-disciplinary team working.

Working Hours:

37.5 per week but 30 hours per week would be considered.

Monday – Friday 08:00 – 16:00 or 09:00 – 17:00 potentially negotiable.

Please note:

From 1 April 2022 it will become a legal requirement for health and social care staff who have direct, face-to-face contact with patients/service users to provide evidence of a complete course of an authorised COVID-19 vaccine unless they are exempt.

This requirement also extends to non-clinical staff not directly involved in patient/service user care, but who may have direct, face-to-face contact with patients/service users in the course of their work.
